What is Acute Respiratory Distress Syndrome (ARDS)?

Acute respiratory distress syndrome (ARDS) is severe hypoxemic respiratory failure that develops when permeability of pulmonary capillaries increases and fluid leaks into the alveoli due to various causes.

It is classified as mild, moderate, or severe according to the Berlin definition. The PaO2/FiO2 ratio is used to determine severity. It is most commonly seen after sepsis, pneumonia, and trauma.

Treatment cornerstones include low tidal volume protective mechanical ventilation, prone positioning, and fluid restriction. Mortality remains high.

Symptoms

Rapidly progressive severe shortness of breath
Tachypnea and use of accessory respiratory muscles
Cyanosis and hypoxemia
Diffuse bilateral crackles
Confusion and agitation
Low oxygen saturation

Risk Factors

Sepsis and severe infection
Aspiration pneumonia
Severe trauma and multiple fractures
Massive blood transfusion
Pancreatitis
Inhalation injury

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Call for emergency help for rapidly worsening shortness of breath and dropping oxygen levels
  • Go to the hospital immediately for sudden respiratory deterioration after fever and cough
  • Call 911 for breathing difficulty accompanied by altered consciousness

Treatment Methods

Low tidal volume protective mechanical ventilation (6 ml/kg)

Prone positioning (at least 12-16 hours per day)

Neuromuscular blocking agents (in severe cases)
