What is ST Elevation Myocardial Infarction (STEMI)?

ST elevation myocardial infarction (STEMI) is the result of complete coronary artery occlusion by a thrombus, leading to ischemia of a large area of the heart muscle. Diagnosis is made by ST segment elevation on ECG.

Primary percutaneous coronary intervention (PCI) is the gold standard treatment for STEMI. Door-to-balloon time should not exceed 90 minutes; if it exceeds 120 minutes, fibrinolytic therapy should be considered.

Delay in reperfusion therapy increases myocardial necrosis and the risk of complications such as heart failure, arrhythmias and cardiogenic shock.

Symptoms

Severe chest pain lasting longer than 20 minutes
Pain radiating to the left arm, neck and jaw
Cold sweating and pallor
Severe shortness of breath
Nausea and vomiting
Fear of death and anxiety

Risk Factors

Smoking
Diabetes and hypertension
Hyperlipidemia
Obesity and sedentary lifestyle
Family history of early coronary disease
Male sex and advanced age

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Call 112 immediately if chest pain lasts longer than 20 minutes
  • Do not delay if chest pain is accompanied by cold sweating
  • Patients with known heart disease should go to the ER for new severe pain

Treatment Methods

Emergent primary percutaneous coronary intervention (balloon and stent)

Fibrinolytic therapy (tPA, tenecteplase) if PCI is not feasible

Dual antiplatelet therapy (aspirin + P2Y12 inhibitor)

Anticoagulation with heparin
