What is Acute Kidney Injury (Emergency)?

Acute kidney injury (AKI) is a loss of renal function developing over hours to days and marked by rising creatinine and/or reduced urine output. It is staged by the KDIGO criteria.

AKI is classified as prerenal (volume loss, hypotension), intrinsic (acute tubular necrosis, glomerulonephritis) or postrenal (obstruction).

Treatment includes cause-specific intervention, volume optimisation, avoidance of nephrotoxins, correction of electrolytes and urgent dialysis when needed.

Symptoms

Oliguria (<400 mL/day) or anuria
Malaise, nausea and vomiting
Oedema and weight gain
Dyspnoea from pulmonary oedema
Confusion or lethargy
Arrhythmia from hyperkalaemia

Risk Factors

Sepsis, hypotension
Nephrotoxic drugs (NSAIDs, aminoglycosides, contrast)
Dehydration
Chronic kidney disease
Diabetes and hypertension
Obstructive uropathy (prostate, stones)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Call 112 for marked reduction in urine output
  • Severe vomiting or diarrhoea risks dehydration
  • Sudden deterioration in a known kidney patient requires emergency care
  • Shortness of breath and oedema may indicate need for dialysis
