What is Acute Coronary Syndrome (NSTEMI)?

Non-ST elevation myocardial infarction (NSTEMI) is a form of acute coronary syndrome in which myocardial damage develops as a result of partial coronary artery occlusion. There is no ST elevation on ECG, but troponin values are elevated.

Patients are risk-stratified using the GRACE and TIMI risk scores. High-risk patients are evaluated with an early invasive strategy.

Dual antiplatelet therapy, anticoagulation and early coronary angiography are the main treatment approaches. Risk stratification determines the timing of treatment.

Symptoms

Tightness or pressure sensation in the chest
Chest pain on exertion or at rest
Shortness of breath
Sweating and nausea
Pain radiating to the arm or jaw
Weakness and fatigue

Risk Factors

Prior coronary artery disease
Diabetes mellitus
Hypertension and hyperlipidemia
Smoking
Family history of early coronary disease

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If chest pain lasts more than 20 minutes, call 112 immediately
  • Patients with known coronary disease should present urgently for new or worsening pain
  • Do not delay if chest pain is accompanied by shortness of breath and cold sweating

Treatment Methods

01
Dual antiplatelet therapy with aspirin and a P2Y12 inhibitor
02
Anticoagulation with low-molecular-weight heparin
03
Early or delayed coronary angiography based on risk classification
04
Decision for stent implantation or bypass surgery
05
Secondary prevention with beta-blocker, statin and ACE inhibitor
