What is Acute Abdominal Pain?

Acute abdominal pain is a clinical syndrome of sudden, severe pain that may indicate intra-abdominal pathology requiring surgery. Appendicitis, perforated ulcer, bowel obstruction and mesenteric ischaemia are among the most common causes.

In women, ectopic pregnancy and ovarian torsion are also life-threatening causes. Diagnosis is reached by combining clinical examination, laboratory tests and imaging.

Symptoms

Sudden severe abdominal pain
Board-like rigidity of the abdomen (muscular guarding)
Nausea, vomiting and inability to pass gas or stool
Fever and chills
Abdominal swelling and tenderness
Bloody stool or urine

Risk Factors

Previous abdominal surgery
Hernia and intestinal adhesions
Gallstones and kidney stones
Peptic ulcer disease and NSAID use
Long-standing constipation
Pregnancy (risk of ectopic pregnancy)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Call emergency services for sudden board-like abdominal pain
  • Go to the emergency department for severe abdominal pain with fever
  • Do not delay if accompanied by bloody vomiting or bloody stools
  • Sudden abdominal pain in pregnancy needs urgent assessment for ectopic pregnancy

IV fluid resuscitation and pain management (morphine does not mask diagnosis)

Blood tests, urinalysis and ultrasound or CT imaging

Emergency surgery for appendicitis, perforation or obstruction
