What is Yoga-Based Physical Therapy for Low Back Pain?

Yoga-based physical therapy is a complementary approach used for chronic (>12 weeks) non-specific low back pain. Recommended in international guidelines (ACP 2017, NICE 2016, ACR/AAS) as a non-pharmacological treatment for chronic low back pain.

Yoga styles: Iyengar yoga (precise alignment, props), Hatha yoga (slower, beginner), Viniyoga (one-on-one, modified), Restorative yoga (passive, supportive). Therapy components: physical postures (asanas), breathing techniques (pranayama), mindfulness/meditation, body awareness.

Evidence-based protocol: 60-90 min/session, 1-2 days/week, 12 weeks (24 sessions). Cochrane review (2017): low-moderate evidence — clinically significant pain reduction (ES 0.4) and functional improvement (ES 0.3). Annals Internal Medicine RCT (Saper 2017): yoga = physical therapy, both more effective than education alone. Beneficial postures: cat-cow, child's pose, downward dog, sphinx, locust, bridge, supine twist. Avoid: full forward bend, deep backbend, severe inversion.
