What is Physical Therapy for Low Back Pain?

Physical therapy for low back pain is a multi-component treatment process planned according to the cause and stage of pain. In acute low back pain, early mobilization and pain control are at the forefront, while active exercise and behavior change become important in chronic low back pain.

Physical therapy methods include physical agent modalities such as TENS, ultrasound, hot-cold application, laser and magnetic field therapy, along with manual therapy (mobilization, manipulation, myofascial release) and exercise therapy.

Randomized controlled trials show that active exercise programs are more effective than passive modalities in the long term. Patient education and reduction of fear-avoidance beliefs are an integral part of treatment.

Symptoms

Acute or chronic pain in the low back region
Morning stiffness and loss of flexibility
Pain or numbness radiating to the leg
Decreased walking and sitting tolerance
Pain increasing with bending forward and movement
Restriction of daily living activities

Risk Factors

Sedentary lifestyle and weak core muscles
Prolonged sitting or heavy lifting
Obesity and poor posture
Psychological stress and depression (for chronic pain)
Low socioeconomic status and job dissatisfaction
Smoking, which impairs vascular structure and disc nutrition

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • For evaluation in initial treatment
  • If neurological symptoms are present
  • If pain worsens with exercise
  • If bowel-bladder control is impaired (emergency)
  • If there is no response to 6 weeks of treatment
