What is Myofascial Pain Syndrome?

A regional musculoskeletal pain disorder driven by myofascial trigger points (MTrPs) within taut muscle bands.

Active trigger points produce spontaneous pain and referred pain in predictable patterns; latent trigger points cause dysfunction without spontaneous pain.

Perpetuating factors include postural overload, repetitive strain, muscle ischemia, and emotional stress.

Distinct from fibromyalgia: regional rather than widespread, with identifiable trigger points.

Symptoms

Localized deep aching muscle pain with predictable referred patterns
Palpable taut band with exquisitely tender trigger point
Local twitch response upon snapping palpation of the trigger point
Restricted range of motion of affected muscle
Autonomic phenomena: sweating, pilomotor response, temperature change
Sleep disturbance and secondary deconditioning

Risk Factors

Repetitive strain or sustained muscular contraction (computer work, driving)
Postural dysfunction and muscle imbalance
Acute muscle overload or trauma
Nutritional deficiencies (vitamin D, B12, iron, magnesium)
Poor sleep and emotional stress
Hypothyroidism or other endocrine contributors

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Failure of self-care after 4–6 weeks of conservative measures
  • Progressive weakness, neurologic deficit, or red flag symptoms
  • Persistent interference with sleep, work, or daily activities
