What is Wheelchair Assessment and Prescription?

A wheelchair is more than a mobility device. It is the user's primary postural support, base for activities of daily living, and tool for community participation. Inappropriate wheelchair prescription contributes to pressure injury, contractures, repetitive strain in propulsion, and functional withdrawal.

Comprehensive assessment evaluates the user's diagnosis and prognosis, postural needs, transfer ability, hand and trunk function, and home and community accessibility. Body measurements include hip width, thigh length, lower leg length, back support height, and elbow position to size the frame and seating system accurately.

Wheelchair categories include manual rigid and folding frames, lightweight propulsion-efficient chairs, tilt-in-space and reclining frames for postural management, and power chairs with various drive control options. Cushion selection considers pressure injury risk and microclimate, while back supports balance posture and trunk freedom. Trial use and follow-up adjustment confirm fit and function.

Symptoms

Pressure injury or skin redness with current chair
Difficulty with self-propulsion
Pain or fatigue during wheelchair use
Postural collapse during sitting
Inability to transfer safely from current chair

Risk Factors

Spinal cord injury or progressive neurologic disease
Lower limb amputation
Severe arthritis or weakness
Inadequate prior wheelchair prescription
Limited access to seating specialty service

  • When current chair causes skin or comfort problems
  • When function declines and chair no longer fits
  • When weight or postural needs change
  • When transfers become unsafe
  • When new home or community demands appear
