What is Ulnar Neuropathy (Cubital Tunnel Syndrome)?

Ulnar neuropathy most often develops as a result of compression of the ulnar nerve in the cubital tunnel behind the elbow. It is the second most common entrapment neuropathy after carpal tunnel syndrome and is frequently seen in office workers, those who keep their elbows bent for prolonged periods, or those who rest their elbows on hard surfaces.

As a result of nerve involvement, sensory changes occur in half of the 4th finger and the 5th finger. In advanced cases, weakness and atrophy of the intrinsic hand muscles may develop; fine motor skills are impaired and grasping objects becomes difficult.

Clinical examination, electromyography (EMG), and nerve conduction studies play a key role in diagnosis. Conservative measures are appropriate in early stages, while surgical decompression may be considered in advanced cases.

Symptoms

Numbness and tingling in the ring and little fingers
Pain or tenderness at the elbow
Worsening symptoms when the elbow is bent for long periods
Weakness in hand muscles and reduced grip strength
Impaired fine motor skills (buttoning, writing)
Atrophy of intrinsic hand muscles in advanced cases (claw hand)
Waking at night due to numbness

Habit of keeping the elbow bent for long periods
Resting the elbow on hard surfaces
Occupations requiring repetitive elbow movements
Diabetes and metabolic neuropathies
Elbow trauma and fractures
Arthritis and elbow joint deformities
Obesity and smoking

  • If there is constant numbness or weakness in the fingers
  • If a noticeable decrease in hand skills is observed
  • If muscle wasting becomes visible
  • If sleep is disrupted by numbness
  • If symptoms progress despite conservative measures
