What is Trigger Point Injection?

Trigger point injection is a method of injecting hyperirritable focal points (trigger points) in skeletal muscle directly with local anaesthetic, saline, corticosteroid or botulinum toxin.

Trigger point identification: hard, tender area on palpation, taut band sensation in the muscle, pain radiating to the typical referral zone with palpation.

It is used in myofascial pain syndrome, chronic neck and low back pain, tension headache and fibromyalgia; high success rate with diagnostic-therapeutic combination.

Symptoms

Localised, deep muscle pain
Limitation of range of motion
Tender taut band on palpation
Referred pain (occipital pain in trapezius, headache)
Pain not responding to muscle relaxant
Muscle weakness and weakness in fine movements

Risk Factors

Chronic muscle overload and poor posture
Repetitive injury and sports trauma
Stress and anxiety (muscle tension)
Sleep disorder and unrefreshing sleep
Vitamin D, B12 and magnesium deficiency
Concurrent fibromyalgia or chronic pain syndrome

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Inadequate response to muscle pain and conservative treatment
  • Reduction of activity and restriction of daily life
  • Spreading muscle pain accompanied by referred pain
  • Acute increase of chronic pain

Treatment Methods

01
Trigger point identification (palpation, sonographic guidance)
02
Lidocaine 1% (1 to 2 mL) or bupivacaine 0.25% injection
03
Saline (dry needling) alternative
04
Corticosteroid addition in selected cases (limited dose)
05
Botulinum toxin in chronic refractory cases
06
Combination with stretching exercises and physical therapy
