What is Pre-Total Knee Arthroplasty Prehabilitation?

Prehabilitation is a structured pre-operative program designed to optimize physical and psychological readiness for surgery.

Components include resistance training, range of motion exercises, aerobic conditioning, gait training, and patient education.

Programs typically run 4–8 weeks before scheduled TKA, supervised by physical therapists.

Goals include increasing quadriceps strength, improving range of motion, optimizing weight, and reducing surgical anxiety.

Education covers expectations, post-operative protocol, pain management, and home preparation.

Evidence demonstrates faster early recovery, shorter hospital stay, and improved functional outcomes at 3 months.

Symptoms

Severe knee osteoarthritis pain limiting daily activities
Functional limitations: difficulty climbing stairs, walking distances
Knee instability or buckling
Decreased range of motion (flexion contracture, limited flexion)
Quadriceps weakness from disuse
Sleep disturbance from nocturnal pain
Failed conservative management (NSAIDs, injections, weight loss, physical therapy)

Risk Factors

End-stage knee osteoarthritis on imaging (Kellgren-Lawrence grade III–IV)
Failed conservative therapy >6 months
Functional decline impacting quality of life
Co-existing comorbidities (diabetes, obesity, cardiac disease) requiring optimization
Pre-operative quadriceps weakness (predictor of slower recovery)
Anxiety or depression about surgery
Limited social support post-operatively

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Patients with end-stage knee osteoarthritis scheduled for TKA should be referred for prehabilitation evaluation.
  • Acute exacerbation of knee pain or instability before surgery requires reassessment.
  • New-onset cardiac, respiratory, or metabolic symptoms before surgery need medical optimization.
  • Significant weight loss or gain may necessitate prosthesis sizing reconsideration.
  • Anxiety or unrealistic expectations should prompt psychological consultation.

Treatment Methods

01
Pre-operative assessment of strength, range of motion, gait, and functional status.
02
Quadriceps strengthening exercises (open and closed kinetic chain).
03
Range of motion exercises emphasizing knee extension and flexion.
04
Aerobic conditioning: stationary cycling, aquatic therapy.
05
Gait training and balance exercises.
06
Education on post-operative protocol, pain management, and home preparation.
07
Optimization of medical comorbidities: diabetes, hypertension, anemia, weight management.
08
Smoking cessation if applicable.
09
Anxiety reduction techniques: relaxation, mindfulness, peer support.
10
Multidisciplinary planning: orthopedic surgeon, physiatrist, physical therapist, anesthesiologist.
