What is Temporomandibular Joint Rehabilitation?

Temporomandibular disorders (TMD) include myofascial pain, disc displacement (with or without reduction), and degenerative joint disease.

TMJ rehabilitation is a conservative, evidence-based first-line approach for most TMD patients.

Components include patient education, posture correction, jaw exercises, manual therapy, modalities, and habit modification.

It is often combined with dental occlusal management, splint therapy, and stress reduction techniques.

Rehabilitation programs typically last 6–12 weeks with home exercise progression.

Outcomes are best when initiated early and integrated with dental and behavioral interventions.

Symptoms

Pain in the jaw, face, ear, or temple region
Clicking or popping sounds with jaw movement
Limited mouth opening or jaw locking
Headaches, especially temporal
Difficulty chewing tough foods
Bruxism (teeth grinding), especially nocturnal
Pain on palpation of masseter, temporalis, and pterygoid muscles

Risk Factors

Bruxism and clenching (often stress-related)
Dental malocclusion
Trauma to the jaw or face
Joint hypermobility
Female sex (2-3× more common)
Poor cervical and head posture
Comorbid headache, fibromyalgia, or chronic pain conditions

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Persistent jaw pain >2 weeks unresponsive to self-care warrants evaluation by dentist or physiatrist.
  • Acute trauma with jaw deformity or inability to close mouth requires emergency evaluation.
  • Locked jaw (closed lock) lasting >24 hours needs urgent specialist assessment.
  • Ear pain or fullness without evident infection should consider TMD.
  • Sudden weight loss due to feeding difficulty requires multidisciplinary evaluation.
