What is Temporomandibular Joint Dysfunction (Detailed)?

Temporomandibular disorders (TMD) affect 5-12% of adults, with female predominance (3:1) and peak incidence in young adults. The DC/TMD (Diagnostic Criteria for TMD) classifies them into Axis I (physical: muscle disorders, disc displacement, arthralgia, headache attributed to TMD) and Axis II (psychosocial: pain disability, depression, somatization).

Disc displacement (with or without reduction) presents with clicking, locking and limited opening. Arthralgia/osteoarthritis causes pain and crepitus. Myofascial pain is the most common subtype with masticatory muscle tenderness. Bruxism (sleep or awake) is a major perpetuating factor identified by tooth wear, masseter hypertrophy and morning jaw stiffness.

Diagnostic workup includes structured DC/TMD examination, panoramic radiograph, CBCT for bony changes and MRI for soft tissue and disc position. Management follows a stepped-care model: education, self-care, splint therapy, physical therapy, pharmacotherapy and surgical options (arthrocentesis, arthroscopy, open surgery) reserved for refractory cases.

Symptoms

Jaw joint pain on chewing or yawning
Clicking or popping with mouth opening
Locking of the jaw (open or closed)
Limited mouth opening (<35 mm)
Crepitus (grinding sensation)
Headaches in temporal region
Ear pain or fullness without otologic cause
Morning jaw stiffness suggesting bruxism

Risk Factors

Female sex (3:1 ratio)
Bruxism (sleep or awake clenching)
Stress, anxiety and depression
Generalized joint hypermobility
History of jaw trauma or whiplash
Malocclusion or recent orthodontic treatment
Connective tissue disease (rheumatoid arthritis)
Fibromyalgia and chronic widespread pain

When to See a Doctor?

  • Persistent jaw pain >2 weeks
  • Sudden inability to open mouth (closed lock)
  • Severe limitation of opening with pain
  • TMJ pain with associated headache or earache
  • Bruxism with tooth wear and morning stiffness
  • Failed self-care after 2-4 weeks
