What is Teeth Whitening?

Extrinsic staining (coffee, tea, wine, tobacco) is largely removed by scaling/polishing; intrinsic discoloration (age, tetracycline, fluorosis) requires bleaching.

In-office bleaching uses 25–40% hydrogen peroxide for 15–60 minutes per session; home bleaching uses 10–22% carbamide peroxide in custom trays for 2–8 hours daily over 1–4 weeks.

Results depend on tooth color, cause of discoloration and compliance; outcomes typically last 6 months to 2 years depending on dietary habits.

Not suitable during pregnancy/lactation or in patients with extensive restorations, decay or hypersensitivity.

Symptoms

Yellow, brown or grey tooth discoloration
Staining from coffee, tea, red wine, tobacco
Age-related yellowing of dentin
Tetracycline or fluorosis staining (intrinsic)
Dissatisfaction with smile aesthetics
Color mismatch with restorations

Risk Factors

Severe cracked teeth, exposed dentin or untreated caries (may worsen sensitivity)
Large anterior restorations (will not whiten, may need replacement)
History of dentin hypersensitivity
Pregnancy/lactation (limited safety data)
Unrealistic aesthetic expectations
Periodontitis or gingivitis (treat first)

When to See a Doctor?

  • Interest in whitening with existing dental problems
  • Severe post-bleaching sensitivity, gum burns or ulceration
  • Unsatisfactory color response or relapse after bleaching
