What is Gingival Recession (Gum Recession)?

The Cairo 2011 classification groups recessions as RT1 (no interdental loss), RT2 (interdental loss ≤ buccal loss) and RT3 (interdental loss > buccal loss).

Thin gingival biotype, high frenal attachment, tooth malposition and traumatic brushing are major local factors.

Periodontitis causes multi-surface generalized recession; mechanical factors often cause localized buccal recession.

Consequences include dentin hypersensitivity, cervical caries and abrasion, aesthetic concerns and risk of continued attachment loss.

Symptoms

Visible exposure of the tooth root or 'long teeth'
Sensitivity to cold, heat or sweets at the cervical area
V-shaped notches (cervical abrasion) at the gumline
Bleeding or tender gums around receded areas
Unaesthetic smile with uneven gum contour
Root caries on exposed surfaces

Risk Factors

Traumatic brushing with hard-bristled brush
Thin periodontal biotype, bony dehiscence
Tooth crowding or labial protrusion
High frenal attachment pulling gingiva
Periodontitis with attachment loss
Oral piercings, parafunctional habits

Treatment Methods

Manage dentin hypersensitivity: desensitizing toothpaste, fluoride varnish or in-office agents

Root coverage surgery (connective tissue graft, coronally advanced flap) for suitable RT1/RT2 defects

Restoration of cervical caries/abrasion with composite or glass-ionomer
