What is Temporomandibular Joint Disorder?

Temporomandibular joint disorder (TMD) is a heterogeneous group of musculoskeletal and neuromuscular conditions affecting the temporomandibular joint, masticatory muscles and associated structures. The DC/TMD criteria classify TMD into pain-related (myalgia, arthralgia, headache attributed to TMD) and intra-articular subtypes (disc displacement, osteoarthritis).

Multifactorial pathogenesis includes occlusal disharmony, parafunctional habits (bruxism, clenching), trauma, psychological stress and systemic conditions. First-line management is conservative and reversible, with surgery reserved for refractory cases.

Symptoms

Pain in the jaw joint or surrounding muscles
Joint clicking, popping or crepitus
Limited mouth opening (less than 35 mm)
Jaw deviation or deflection on opening
Headaches in temporal region
Earache without otologic disease
Tinnitus, ear fullness or vertigo

Risk Factors

Bruxism and clenching habits
Female gender and age 20-50
Anxiety, depression and chronic stress
Acute or chronic facial trauma
Generalized hypermobility (Ehlers-Danlos)
Rheumatoid arthritis or other inflammatory arthropathy
Malocclusion and previous orthodontic intervention

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Persistent jaw pain over 4 weeks
  • Sudden inability to open or close mouth
  • Recurrent jaw locking
  • Painful clicking interfering with eating or speech
  • Associated headaches or earache
  • Dental wear from bruxism
