What is Subarachnoid Hemorrhage?

Subarachnoid hemorrhage (SAH) is a serious neurological emergency accounting for approximately 5% of all strokes, occurring when blood leaks into the subarachnoid space between the arachnoid and pia mater. The most common cause is rupture of a cerebral aneurysm.

The most distinctive feature of SAH is a thunderclap headache described as the 'worst headache of life' that reaches its maximum within seconds to minutes. This pain may be accompanied by neck stiffness and altered consciousness.

SAH mortality is high; rebleeding following the initial bleed and cerebral vasospasm are the most important complications. Rapid diagnosis and treatment are critical for patient survival and functional recovery.

Symptoms

Sudden-onset severe headache ('worst headache of my life')
Neck stiffness and photophobia
Nausea and vomiting
Confusion or loss of consciousness
Seizure
Double vision or drooping eyelid

Risk Factors

Presence of cerebral aneurysm
Smoking
Uncontrolled high blood pressure
Family history of aneurysm or SAH
Connective tissue disorders such as polycystic kidney disease

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Go immediately to the emergency room in sudden-onset worst-ever headache
  • If headache is accompanied by neck stiffness or altered consciousness
  • If sudden vision disturbance or eyelid drooping develops
  • If unexplained fainting or seizure occurs
