What is Spontaneous Intracranial Hypotension?

Spontaneous intracranial hypotension is a rare but severe headache condition resulting from a spontaneous leak of cerebrospinal fluid (CSF) at the spinal level, leading to low intracranial pressure.

The hallmark feature is an orthostatic headache that worsens within minutes of standing and improves on lying down. Neck pain, nausea, dizziness, tinnitus, and sometimes consciousness changes may accompany.

Diagnosis is made by detailed history, brain MRI (dural thickening, sagging signs), and spinal imaging. Treatment is mostly aimed at sealing the leak.

Symptoms

Headache that worsens upright and improves lying down
Neck stiffness and pain
Nausea and vomiting
Tinnitus and hearing changes
Double vision
Dizziness
Rare consciousness changes

Risk Factors

Connective tissue disorders (Marfan, Ehlers-Danlos)
Spinal degeneration
Prior lumbar puncture
Minor trauma
Sudden exertion or heavy lifting
Female sex
Age 30–50

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Headache that appears upright and resolves on lying down
  • Sudden-onset neck pain and hearing changes
  • Accompanying nausea, vomiting, and visual disturbance
  • Symptoms severe enough that the patient cannot stand
  • Recurrent attacks
