What is Sciatica?

Sciatica is a condition characterized by pain, numbness, and tingling starting from the lower back and extending to the buttock, leg, and foot, caused by involvement of the sciatic nerve, the largest nerve in the body. It is not a disease in itself but a symptom of an underlying problem.

The most common cause is lumbar disc herniation. Compression of the nerve root at the L4-L5 or L5-S1 level leads to sciatic pain. Piriformis syndrome, spinal stenosis, and spondylolisthesis are among the other causes.

The vast majority of cases improve within 6-12 weeks with conservative treatment. If there is leg weakness or impairment of bladder-bowel control, urgent specialist evaluation is required.

Symptoms

Sharp pain starting from the lower back and extending to the back of the buttock and leg
Numbness, tingling, or burning sensation in the leg
Weakness in the foot or foot drop (in severe cases)
Pain worsening with forward bending, sitting, or prolonged walking
Pain intensifying with coughing or sneezing
Deep, dull pain in the buttock

Risk Factors

Lumbar disc herniation
Sedentary lifestyle and prolonged sitting
Overweight
Heavy lifting
Advanced age and disc degeneration
Diabetic neuropathy

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If significant weakness in the leg or foot drop has developed
  • If there is a problem with urine or stool control (nerve compression is emergency)
  • If pain lasts longer than 4-6 weeks and does not resolve with rest
  • If fever or night sweats accompany
  • If there is numbness in both legs

Staying active and exercises recommended by the physician (bed rest is not recommended)
