What is Sacroiliac Joint Dysfunction?

Sacroiliac joint dysfunction is a syndrome arising from hypomobility, hypermobility or inflammatory disorders of the joint between the sacrum and the iliac bone. It is responsible for 15-30% of chronic mechanical low back pain.

Pain is typically localised to the buttock and the posterior superior iliac spine; it may radiate to the upper posterior thigh, but rarely extends below the knee. Onset is more common after pregnancy, trauma or spinal fusion.

On clinical examination FABER, distraction, compression and thigh-thrust tests are used; positivity of three or more provocative tests supports the diagnosis. Diagnostic injection guided by fluoroscopy is the gold standard.

Symptoms

Buttock and posterior superior iliac spine pain
Increased pain on prolonged sitting, standing or stair climbing
Pain referring to the upper posterior thigh
Difficulty rolling over in bed
Pain in transitional movements
Local tenderness on palpation
Limp in walking

Risk Factors

Pregnancy and recent delivery
Trauma (fall, motor vehicle accident)
Lumbar spinal fusion (especially long segment)
Leg length discrepancy
Chronic stress and biomechanical imbalance
Inflammatory rheumatic diseases (axial spondyloarthritis)
Older age and degeneration

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Persistent buttock pain not responsive to treatment
  • Severe pain on rolling over and transitional movements
  • Pain not improving with conservative therapy
  • Limitation of daily activities
