What is Root Canal Treatment (Endodontic Therapy)?

Root canal treatment (endodontic therapy) is a dental procedure to remove infected, inflamed, or necrotic pulp tissue from the tooth's pulp chamber and root canals, followed by cleaning, shaping, disinfection, and obturation of the canal system.

Modern endodontics uses nickel-titanium rotary or reciprocating instruments, electronic apex locators, irrigation with sodium hypochlorite and EDTA, and three-dimensional obturation with gutta-percha and bioceramic sealers under rubber dam isolation and dental operating microscope.

Indications include irreversible pulpitis (severe spontaneous pain), pulp necrosis with apical periodontitis, dental trauma with pulp involvement, deep caries approaching the pulp, and elective endodontics for prosthetic restoration. Success rate exceeds 95% with proper technique and case selection.

Symptoms

Severe spontaneous toothache, especially at night
Prolonged sensitivity to hot or cold lasting minutes
Pain on biting or chewing (apical periodontitis)
Tooth discoloration (pulp necrosis)
Swelling or abscess in surrounding gum or face
Sinus tract draining pus on gum
Deep caries or fracture exposing pulp

Risk Factors

Untreated deep dental caries
Dental trauma (fracture, luxation, avulsion)
Repeated dental procedures on same tooth
Cracked tooth syndrome
Periodontal disease with pulp involvement
Bruxism causing tooth wear and pulp exposure
Failed previous endodontic treatment requiring retreatment

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Severe persistent toothache
  • Sensitivity to hot/cold lasting more than 30 seconds
  • Pain on biting or pressure
  • Visible swelling or pus drainage
  • Tooth discoloration after trauma
  • Recurrent dental pain after filling or crown
  • Suspected pulp involvement on dental imaging

Treatment Methods

01
Local anesthesia and rubber dam isolation
02
Access cavity preparation through occlusal or lingual surface
03
Pulp removal and canal exploration with hand and rotary files
04
Working length determination with electronic apex locator and radiographs
05
Canal cleaning and shaping with rotary nickel-titanium instruments
06
Irrigation with sodium hypochlorite, EDTA, and chlorhexidine
07
Three-dimensional obturation with gutta-percha and bioceramic sealer
08
Coronal restoration with composite or crown for long-term success

Teeth whitening lightens tooth color using hydrogen peroxide or carbamide peroxide gels. In-office (professional) bleaching uses higher concentrations with light activation for faster results; home bleaching uses custom trays with lower concentrations over 1–4 weeks. Safety depends on healthy teeth, correct concentration, isolation of gums and transient sensitivity management.
