REM Sleep Behavior Disorder

What is REM Sleep Behavior Disorder?

REM sleep behavior disorder (RBD) is a parasomnia in which the muscle paralysis (atonia) that should normally occur during REM sleep is impaired, so that the person physically acts out their dreams.

Patients may shout, swing their arms as if fighting, fall out of bed, or unintentionally injure their bed partners during sleep. When awakened, the dream is usually clearly remembered.

RBD may be an early sign of synucleinopathy-spectrum neurodegenerative diseases such as Parkinson disease, dementia with Lewy bodies, and multiple system atrophy. Long-term neurological follow-up is recommended for affected individuals.

Symptoms

Shouting, crying, or swearing during sleep
Swinging arms in bed, throwing punches, kicking
Falling out of bed and injuring oneself
Vividly remembering the dream upon awakening
Inability of the partner to sleep, or partner injury
Increasingly frequent night-time events

Risk Factors

More common in men older than 60
Parkinson disease or parkinsonism
Use of antidepressants (especially SSRIs/SNRIs)
Narcolepsy
Family history of RBD

  • If a partner or roommate reports complex night-time behaviors
  • If Parkinson disease or dementia is suspected
  • If it is desirable to assess whether current antidepressant therapy is triggering the disorder
