What is Rehabilitation: Sarcopenia Exercise Therapy?

Sarcopenia involves quantitative loss of muscle mass plus reduced muscle strength or physical performance. EWGSOP2 algorithm uses SARC-F questionnaire screening, grip strength measurement (under 27 kilograms in men, 16 in women), confirmed by appendicular skeletal muscle index from DXA or BIA below sex-specific cutoffs, and severity classified by gait speed under 0.8 meter per second.

Pathogenesis involves anabolic resistance with aging, chronic low-grade inflammation, mitochondrial dysfunction, neuromuscular junction degradation, and reduced satellite cell function. Disuse, malnutrition with insufficient protein, vitamin D deficiency, and chronic disease accelerate loss. Falls, disability, hospitalization, and mortality all increase.

Progressive resistance training is the cornerstone: 2 to 3 sessions weekly with 8 to 10 exercises targeting major muscle groups at 60 to 80 percent of one-repetition maximum, 8 to 12 repetitions, 2 to 3 sets, with 48 to 72 hours rest between sessions. Combined with whey protein 25 to 40 grams within 30 minutes post-exercise, leucine-enriched supplements, vitamin D 800 to 2000 IU daily, and treatment of underlying chronic disease.
