What is Rehabilitation: Hypertension Exercise Therapy?

Hypertension affects approximately 30 percent of adults globally and is a leading risk factor for stroke, myocardial infarction, heart failure, and kidney disease. Exercise reduces blood pressure through improved endothelial function, decreased peripheral resistance, reduced sympathetic activity, and weight loss when accompanied by dietary changes.

Recommended aerobic exercise is 150 minutes weekly of moderate intensity (40 to 60 percent of heart rate reserve) or 75 minutes of vigorous activity, distributed across most days. Walking, cycling, swimming, and dancing are preferred modalities. Resistance training two to three days weekly with light to moderate loads (50 to 70 percent of one-repetition maximum) at 8 to 10 exercises provides additional benefit.

Pre-exercise evaluation excludes severe uncontrolled hypertension (above 180/110), unstable cardiovascular disease, and end-organ damage. Stress testing is recommended for high-risk patients before vigorous exercise. Medications like beta-blockers blunt heart rate response; rate of perceived exertion guides intensity in such patients. Avoid Valsalva maneuver and isometric holds in severe hypertension.
