What is Rehabilitation: Foot Orthoses and Insoles?

Foot orthoses are devices placed inside footwear that modify foot mechanics and pressure distribution. They range from prefabricated over-the-counter insoles to custom-molded orthoses fabricated from cast or 3D scan of the foot. Materials include ethylene-vinyl acetate, polypropylene, carbon fiber, and viscoelastic polymers tailored to indication.

Common indications include plantar fasciitis (medial arch support reduces tension on plantar fascia), posterior tibial tendon dysfunction (medial heel wedge with arch support), metatarsalgia (metatarsal pad proximal to second to fifth metatarsal heads), Morton's neuroma (metatarsal pad and forefoot rocker), and diabetic foot ulcer prevention (total contact insoles redistributing pressure).

Prescription requires gait analysis, foot type assessment (planus, cavus, neutral), pressure mapping in selected cases, and consideration of footwear compatibility. Adaptation period of 2 weeks with gradual wear time increase prevents discomfort. Replacement annually for athletic use, every 18 to 24 months for daily wear. Diabetic insoles inspected every 3 months.
