What is Rehabilitation After Rotator Cuff Repair?

Rehabilitation after rotator cuff repair is built on the principle that tendon-bone healing takes about six weeks. Tear size, repair quality and graft use determine the pace of rehabilitation.

The program usually consists of four phases: Phase 1 (immobilization, weeks 0-6), Phase 2 (active-assisted, weeks 6-12), Phase 3 (strengthening, weeks 12-16) and Phase 4 (return to sport, weeks 16-24).

In the early period passive motion protects the repair, whereas active motion may compromise its integrity. With very large tears the protocol is more conservative and longer; the risk of re-tear can range from 20% to 90%.

Symptoms

Postoperative shoulder pain
Restricted motion
Muscle weakness
Night pain
Shoulder stiffness
Sleep disturbance
Limitation of daily activities

Risk Factors

Large tear size
Advanced age
Diabetes (impaired healing)
Smoking
Lack of prehabilitation
Trauma during the active period
Non-adherence to the rehabilitation protocol
