What is Rehabilitation After Anterior Cruciate Ligament Reconstruction?

Rehabilitation after ACL reconstruction is one of the most important determinants of functional outcome. A phased program (phases 1-5) is applied, with specific goals to be reached in each phase.

Modern accelerated protocols favor early loading, early range of motion and closed-kinetic-chain exercises. Goals are return to daily activity in 4-6 weeks and return to sport in 4-6 months.

Return-to-sport criteria include full range of motion, quadriceps strength reaching 90%, symmetry on functional tests (hop tests) and psychological readiness. Early return increases the risk of re-rupture.

Postoperative knee pain
Restricted flexion
Loss of extension
Quadriceps atrophy
Sense of knee instability
Swelling
Gait asymmetry

Risk Factors

Early return to sport
Poor neuromuscular control
Suboptimal graft placement
Non-adherence to rehabilitation
Concurrent meniscal injury
Female athletes (higher re-rupture rate)
Inadequate strength gains
