What is Cardiac MRI Tissue Characterization?

Cardiac MRI tissue characterization is an advanced MR technique that evaluates structural properties of myocardium using T1 mapping T2 mapping extracellular volume calculation and late gadolinium enhancement. These techniques provide quantitative tissue parameters allowing detection of fibrosis edema infiltration and iron overload. The modality has transformed the diagnostic approach to non ischemic cardiomyopathies infiltrative diseases and inflammatory conditions.

T1 mapping measures longitudinal relaxation time of myocardium and detects fibrosis edema and infiltrative diseases. Native T1 elevation suggests fibrosis or edema while reduced T1 indicates fat infiltration or iron deposition. Extracellular volume fraction calculated from pre and post contrast T1 values quantifies interstitial expansion and correlates with histologic fibrosis. T2 mapping specifically identifies myocardial edema and inflammation supporting acute myocarditis diagnosis.

Late gadolinium enhancement detects focal fibrosis with patterns suggesting specific etiologies. Subendocardial enhancement following coronary distribution indicates ischemic cardiomyopathy. Mid wall enhancement suggests dilated cardiomyopathy or sarcoidosis. Subepicardial enhancement points to myocarditis. Diffuse subendocardial enhancement raises concern for amyloidosis. Pattern recognition combined with quantitative parameters guides diagnosis treatment selection and prognosis. Specialized centers integrate these techniques into routine cardiac MR protocols.
