What is Pure Sensory Neuropathy?

Pure sensory neuropathy is a group of diseases that selectively affect the sensory fibers of the peripheral nervous system; motor fibers are largely preserved. Pain, numbness, tingling, and balance disturbance are prominent in patients.

Causes include paraneoplastic syndromes (especially anti-Hu syndrome associated with small cell lung cancer), Sjögren syndrome, vitamin B6 toxicity, HIV, some chemotherapy drugs such as cisplatin, and hereditary neuropathies.

Diagnosis is made with detailed nerve conduction studies, blood tests, autoantibodies, and nerve biopsy when necessary. Treatment is directed at the underlying cause.

Symptoms

Numbness, tingling in hands and feet
Burning-type pain (especially at night)
Balance disturbance and difficulty walking in the dark
Decrease in touch and vibration sensation
Hypersensitivity (allodynia, hyperalgesia)
Muscle strength largely preserved
Frequent falls and loss of dexterity

Risk Factors

Some cancer types (especially small cell lung)
Sjögren syndrome and other autoimmune diseases
Excessive vitamin B6 intake
Some chemotherapy drugs such as cisplatin
HIV and some viral infections
Hereditary sensory neuropathies
Long-term alcohol use

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If progressive numbness or burning pain is present
  • If imbalance and falls have begun
  • If systemic symptoms such as weight loss and weakness accompany
  • New onset in patients with cancer history
  • New symptoms in patients with autoimmune disease diagnosis
