What is Pulpitis?

Pulpitis is inflammation of the dental pulp tissue, the neurovascular core within the tooth. It is classified as reversible (mild inflammation that resolves with removal of the irritant) or irreversible (persistent inflammation that progresses to pulp necrosis without endodontic intervention).

Reversible pulpitis presents with brief, sharp pain to cold or sweet stimuli that subsides quickly. Irreversible pulpitis features lingering, spontaneous, throbbing pain that often worsens at night and may radiate. Common causes include deep caries, traumatic tooth fracture, repeated dental procedures and bacterial microleakage around restorations.

Symptoms

Sharp pain to cold, hot or sweet stimuli
Lingering pain after stimulus removal (irreversible)
Spontaneous, throbbing tooth pain
Pain that worsens when lying down
Difficulty localizing the affected tooth
Tenderness to percussion in advanced disease
Possible referred pain to ear or jaw

Risk Factors

Untreated dental caries
Dental trauma and tooth fracture
Multiple deep restorations
Bruxism with attrition
Cracked tooth syndrome
Periodontal disease with retrograde infection
Diabetes mellitus and immunosuppression

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Persistent toothache lasting more than 24-48 hours
  • Spontaneous nocturnal tooth pain
  • Sensitivity to hot stimuli relieved by cold
  • Severe throbbing pain interfering with sleep
  • Swelling around tooth or facial swelling
  • Pain after recent dental procedure
