What is Pulmonary Rehabilitation: PMR Program?

Pulmonary rehabilitation (PR) is a comprehensive program for patients with chronic respiratory disease, designed to improve physical and emotional condition and promote long-term adherence to health-enhancing behaviors.

Core components: individualized exercise training (aerobic + resistance), patient education, behavioral interventions, and psychosocial support.

Standard duration: 6-12 weeks with 2-3 supervised sessions per week (24-36 sessions total), followed by home-based maintenance program.

Indications include COPD (most common), interstitial lung disease, bronchiectasis, post-COVID syndrome, pre/post-lung transplant, pulmonary hypertension, and chronic asthma with limitation.

Symptoms

Chronic dyspnea on exertion limiting daily activities
Reduced exercise tolerance (decreased 6-minute walk distance)
Frequent COPD exacerbations or hospitalizations
Anxiety or depression related to chronic lung disease
Decreased quality of life and social participation

Risk Factors

Unstable cardiac disease, recent MI without clearance, uncontrolled arrhythmia
Severe orthopedic limitations preventing exercise participation
Active substance abuse, severe cognitive impairment
Lack of motivation or poor adherence to therapy (relative)
Need for continuous high-flow oxygen at home (relative — modify program)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Apply for pulmonary rehab referral after any moderate-severe COPD exacerbation
  • Re-evaluate any patient with progressive dyspnea despite optimized inhaler therapy
  • Schedule baseline 6-minute walk test, spirometry, and dyspnea scale (mMRC, CAT) for individualized prescription
  • Seek pre-transplant assessment for lung transplant candidates

Treatment Methods

Aerobic training: walking, cycling, treadmill at 60-80% peak workload, 20-30 minutes per session, progressing intensity weekly

Resistance training: upper and lower extremity strengthening with 2-3 sets of 8-12 reps for major muscle groups

Education: disease pathophysiology, inhaler technique, action plans, smoking cessation, nutrition, energy conservation, breathing techniques (pursed-lip, diaphragmatic)

Psychosocial support: anxiety/depression screening, support groups, sexual health counseling, advance care planning

Maintenance: home exercise program, community pulmonary rehab groups, telehealth follow-up; integration with vaccination, smoking cessation, optimal medical therapy
