What is Pudendal Neuralgia?

Pudendal neuralgia is a neuropathic pain syndrome characterized by chronic pain in the perineum, genital area, anus, and around the buttocks, resulting from compression or irritation of the pudendal nerve originating from the sacral plexus.

Pain typically worsens with sitting and is relieved by standing or lying down. Burning, electric-shock-like sensations, and numbness are the most commonly described pain qualities. Bladder, bowel, and sexual dysfunction may accompany.

It is a syndrome that is frequently misdiagnosed or diagnosed late; for an accurate diagnosis, a multidisciplinary evaluation involving neurology, urology, and pelvic floor physiotherapy is recommended.

Symptoms

Burning and pain in the perineum or genital area
Pain that worsens with sitting and improves with standing
Pain around the anus
Burning during urination
Pain during intercourse (dyspareunia)
Numbness or hypersensitivity

Risk Factors

Long-term cycling
Pelvic surgery or birth trauma
Chronic constipation
Excessive tension of pelvic floor muscles
Connective tissue weakness
