What is Multiparametric Prostate MRI (PI-RADS)?

Multiparametric prostate MRI (mpMRI) is an advanced MR protocol that combines anatomical (T2-weighted), functional (diffusion-weighted imaging-DWI), and perfusion (dynamic contrast-enhanced-DCE) sequences.

Lesions are scored from 1 to 5 with the PI-RADS (Prostate Imaging Reporting and Data System) v2.1 classification; PI-RADS 4-5 lesions carry a high probability for clinically significant prostate cancer.

It is used in patients on active surveillance to follow progression, in lesion localization before biopsy (MR-fusion biopsy), and in staging before radical prostatectomy.

Symptoms

High or rising PSA level
Suspicious finding on digital rectal examination
Persistent suspicion despite previous negative biopsy
Active surveillance follow-up of prostate cancer
Local staging of prostate cancer

Risk Factors

Gadolinium contrast allergy (rare)
Severe renal failure (NSF risk)
Pacemaker or non-MR-compatible implant
Severe claustrophobia
Large hip prosthesis (artifact)
