What is Cluster Headache?

Cluster headache is a primary headache disorder characterized by extremely severe pain attacks lasting from 15 minutes to 3 hours and recurring 1–8 times per day, centered on one side of the face around the eye.

Attacks occur daily during certain periods (cluster periods) lasting weeks or months, followed by long remission periods. Eye redness, tearing, nasal congestion, and a state of restlessness on the affected side are typical accompanying features.

During a cluster headache attack, the patient often sits up and tries to walk; unlike in migraine, lying still does not relieve the pain. Diagnosis is made by a neurologist; high-flow oxygen and triptans are used to abort the attack.

Symptoms

Unilateral piercing or burning pain around the eye
Eye redness and tearing on the affected side
Runny or stuffy nose
Drooping eyelid and pupillary constriction
Sweating and agitation
Attacks recurring at regular times (especially at night)

Risk Factors

Male sex (3–4 times more common)
Smoking
Family history of cluster headache
Alcohol (triggers attacks during cluster periods)
Sleep irregularity

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Severe recurring unilateral headache around the eye
  • Headache accompanied by eye redness and nasal symptoms
  • Routine analgesics ineffective
  • Headaches that wake the patient at the same time every night

Treatment Methods

Acute attack: high-flow oxygen therapy (7–12 L/min via mask)

Triptan (sumatriptan) subcutaneous or nasal injection

Verapamil or corticosteroids to shorten the cluster period

Lithium or valproate (in resistant chronic cases)

Smoking and alcohol cessation
