What is Preprosthetic Rehabilitation?

Preprosthetic rehabilitation is the process applied to prepare the residual limb (stump) for prosthesis use, ensure cardiovascular conditioning, and initiate psychological adaptation after amputation. It typically lasts 3-8 weeks postoperatively.

Goals: shaping and desensitization of the stump, preservation of joint range of motion, cardiovascular endurance, phantom pain management, optimization of overall muscle strength, and psychological readiness of the patient for prosthesis use.

Contracture (especially knee flexion in transtibial amputations and hip flexion in transfemoral amputations) is the most common complication and an important goal of treatment.

Symptoms

Stump deformity and edema
Phantom pain and sensations
Tendency for joint contracture
Muscle weakness and loss of conditioning
Difficulty with psychological adjustment
Balance disorders

Risk Factors

Vascular amputation (diabetes, PAD)
Traumatic amputation
Lack of early mobilization
Inadequate postoperative pain management
Elderly and chronic illnesses
Late initiation of rehabilitation

  • On development of phantom pain
  • On suspicion of contracture
