What is Posterior Reversible Encephalopathy Syndrome (PRES)?

Posterior reversible encephalopathy syndrome PRES is an acute neurological syndrome characterized by vasogenic edema, especially in the parieto-occipital region.

It is most often associated with hypertensive crisis, eclampsia, immunosuppressive medications, autoimmune diseases and renal failure.

With rapid recognition and treatment of the trigger it is often reversible.

Symptoms

Acute or subacute headache
Generalized tonic-clonic seizures
Visual disturbances: blurred vision, hemianopia, cortical blindness
Confusion and altered consciousness
Focal neurological deficit

Risk Factors

Severe hypertension or hypertensive crisis
Pre-eclampsia, eclampsia and HELLP syndrome
Immunosuppressive medications such as cyclosporine and tacrolimus
Autoimmune diseases such as SLE and vasculitis
Renal failure and chemotherapy

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Sudden severe headache, seizure or visual loss — emergency
  • Hypertensive crisis with neurological symptoms
  • Eclampsia or pre-eclampsia neurological symptoms
  • Acute change in consciousness in immunosuppressive medication use
