What is Post-Polio Syndrome — Advanced Management?

Post-polio syndrome (PPS) is a clinical condition that develops in survivors of paralytic poliomyelitis 15-40 years after the acute illness. It is characterized by new or worsening muscular weakness, fatigue, joint and muscle pain, atrophy and decreased function, in muscles previously affected and sometimes in unaffected ones.

The pathophysiology relates to chronic overload of compensatory motor units that earlier reinnervated denervated muscles, leading to motor neuron dysfunction, axonal degeneration and progressive neuromuscular junction failure. Risk factors include severity of original poliomyelitis, female sex, age, and physical overuse.

Diagnosis is clinical, supported by EMG, MRI and exclusion of other causes (radiculopathy, ALS, neuropathy). Advanced management combines paced exercise (avoiding overuse), energy conservation, weight management, orthoses, assistive devices, sleep-disordered breathing therapy, pain control, and patient education. Pharmacotherapy is supportive (analgesics, modafinil for fatigue, treatment of comorbid OSA).

Sleep-disordered breathing
Dysphagia and bulbar weakness
Atrophy in previously affected muscles

Risk Factors

History of acute paralytic polio
Severe initial paralysis
Female sex
Older age and longer recovery
Physical overuse
Obesity and deconditioning
Comorbid sleep apnea
