What is Post-COVID Rehabilitation?

Post-COVID rehabilitation, also known as long COVID rehabilitation, addresses the persistent multisystem symptoms that affect 10–30% of COVID-19 survivors beyond 12 weeks of acute illness.

The condition presents with fatigue, post-exertional malaise (PEM), dyspnoea, cognitive dysfunction (brain fog), autonomic dysregulation (POTS), musculoskeletal pain, sleep disturbances and mental health problems.

Mechanisms involve persistent immune dysregulation, microvascular and endothelial dysfunction, mitochondrial impairment, autonomic nervous system dysfunction and possibly viral persistence in tissues.

Rehabilitation must be individualised and pacing-based. Aggressive graded exercise can worsen symptoms in patients with PEM (myalgic encephalomyelitis-like presentation). Multidisciplinary assessment guides intervention.

Symptoms

Profound fatigue and post-exertional malaise (worsening 12–48 hours after activity)
Dyspnoea on exertion, chest tightness, persistent cough
Cognitive dysfunction: brain fog, poor concentration, memory complaints
Postural orthostatic tachycardia syndrome (POTS): tachycardia, dizziness on standing
Muscle and joint pain, generalised weakness, deconditioning
Sleep disturbances, anxiety, depression, post-traumatic stress
Anosmia and dysgeusia (persistent smell and taste alterations)

Risk Factors

Severe acute COVID-19 (ICU stay, mechanical ventilation)
Female sex (2:1 ratio for long COVID)
Pre-existing autoimmune, mental health or chronic pain conditions
Older age and multiple comorbidities (in severe cases)
Persistent viral antigens or autoimmunity in tissues
Vaccination status (unvaccinated higher risk)
Reinfection and SARS-CoV-2 variant

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Persistent symptoms beyond 12 weeks despite recovery from acute infection
  • Chest pain, severe dyspnoea or syncope require cardiopulmonary evaluation
  • Worsening cognitive function impairing work or daily life — neuropsychology referral
  • Severe fatigue with PEM — avoid aggressive exercise; pacing-based rehab
  • Mood disturbance, suicidal ideation — urgent mental health support
