What is Post-Concussion Syndrome Rehabilitation?

Post-concussion syndrome (PCS) is a constellation of headache, dizziness, cognitive complaints, mood disturbance and sleep problems persisting more than 3 weeks after mild traumatic brain injury (mTBI). It is common in athletes, where return-to-play protocols are essential.

Rehabilitation follows a domain-based approach: 1) cervicogenic — manual therapy and exercise, 2) vestibulo-ocular — VRT and oculomotor exercises, 3) cognitive — neuropsychological assessment and cognitive training, 4) psychological — CBT, 5) autonomic — building exercise tolerance.

Whereas absolute rest was once recommended in the early phase, current practice favours graded activity, early submaximal aerobic exercise and domain-specific rehabilitation. The Buffalo Concussion Treadmill Test assesses exercise tolerance. Recovery within 1 month is expected in 80-90% of patients.
