What is Shoulder Rehabilitation After Breast Cancer?

Shoulder problems after breast cancer surgery may occur in 60–70% of patients. Common issues include restricted shoulder motion, scapular dyskinesia, axillary web syndrome (cording), lymphedema and pain.

Rehabilitation is multidisciplinary. Coordination between physiatry, physiotherapy, plastic surgery and oncology is required. Fibrotic changes after radiotherapy may make rehabilitation more challenging.

The early postoperative period (1–6 weeks) focuses on preserving range of motion and ensuring scar/soft tissue health. In the later period, strengthening, functional improvement and lymphedema management come to the fore.

Symptoms

Restricted shoulder motion (especially flexion, abduction)
Pain in the axilla and arm
Axillary web syndrome (cording)
Lymphedema
Weakness of the muscles around the shoulder
Postoperative scar adhesions

Risk Factors

Axillary lymph node dissection
Radiotherapy
Mastectomy (more than breast-conserving surgery)
Development of axillary web syndrome
Inadequate postoperative pain management
Lack of early mobilization

  • Within the first 2 weeks after surgery
  • When restriction of shoulder motion develops
  • When cording is noticed
  • With signs of lymphedema
  • When chronic pain develops
