What is Piriformis Syndrome?

Piriformis syndrome develops when the piriformis muscle compresses the underlying sciatic nerve due to muscle contraction, spasm, or anatomical variation. It is also called 'pseudo-sciatica' because it mimics classical sciatic findings.

Patients complain of deep hip pain, discomfort that worsens with sitting, and sciatica-like symptoms radiating down the back of the leg. Long driving sessions, repetitive running, and trauma may trigger it.

Diagnosis is made by clinical examination, provocation tests, and imaging; differential diagnosis from lumbar disc disease is important. Physical therapy is the cornerstone of treatment.

Symptoms

Deep pain in the hip
Pain radiating to the back of the leg
Discomfort that worsens with prolonged sitting
Worsening with stair climbing and bending
Numbness and tingling in the leg
Pain that becomes more pronounced when driving
Tender spot in the hip

Risk Factors

Prolonged sitting
Repetitive running and athletic activities
Hip trauma
Poor sleep position
Leg length discrepancy
Pelvic muscle imbalance
Incorrect exercise technique

  • If response to conservative treatment is inadequate
