What is Piriformis Muscle Injection?

Piriformis injection is a procedure used in piriformis syndrome to relieve buttock pain, sciatic nerve compression and referred pain to the lower limb. It is performed under ultrasound or fluoroscopic guidance to ensure accurate targeting and avoid injury to the sciatic nerve.

Various agents are used: local anaesthetic alone (for diagnostic purpose), local anaesthetic plus corticosteroid (for therapeutic effect), or botulinum toxin (for refractory cases and chronic spasm).

Effect typically begins within 1-2 weeks and may last 3-6 months. The procedure is combined with conservative treatment in most cases and integrated with patient-specific stretching exercise programmes.

Symptoms

Localised pain at the injection site (transient)
Local oedema
Bruising and minor bleeding
Worsening sciatic nerve symptoms (rare)
Allergic reaction (very rare)
Local anaesthetic systemic toxicity (rare)
Lower limb weakness due to needle misplacement (very rare)

Risk Factors

Failure of conservative therapy
Persistent piriformis spasm
Buttock pain due to chronic compression
Patient with recurrent symptoms
Severe sciatic nerve compression
Pregnancy (relative contraindication)
Coagulation disorder (relative contraindication)
