What is Pilates Rehabilitation for Chronic Low Back Pain?

Pilates is an exercise method developed by Joseph Pilates in the early 1900s, comprising controlled, breath-coordinated movements that strengthen core muscles and improve posture.

Mat pilates and reformer (special apparatus) types are available, and the modified clinical pilates approach is preferred for low back pain.

It has been shown effective in chronic non-specific low back pain in randomised controlled trials and is included in the European Union and National Institute for Health and Care Excellence guidelines.

Symptoms

Mechanical pain in the lumbar region (worse with movement)
Stiffness in the morning lasting 30 to 60 minutes
Increasing pain with prolonged sitting and standing
Pain limited to the lumbar region (no radicular symptom)
Restriction of trunk flexion-rotation
Postural imbalance and muscle imbalance

Risk Factors

Sedentary lifestyle and prolonged sitting work
Poor posture and weak core muscles
Excess weight and abdominal obesity
Pregnancy and postpartum (lumbar lordosis)
Chronic stress and sleep disorder
Repetitive lifting work

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Initial evaluation in low back pain lasting longer than three months
  • Red flags: night pain, weight loss, fever, neurological symptoms
  • Surgery should be excluded before starting pilates
  • Modified programme by physiotherapist supervision is recommended
