What is Pilates in Rehabilitation?

Pilates is a mind-body movement system originally developed by Joseph Pilates that emphasises controlled breathing, neutral spine, core engagement, precise alignment and flowing movement; clinical Pilates adapts these principles to therapeutic goals.

In rehabilitation it is used as low-impact training of deep trunk stabilisers (transversus abdominis, multifidus, pelvic floor, diaphragm), postural muscles, hip and shoulder girdles, often with mat or specialised equipment such as the reformer, Cadillac and chair.

Indications include chronic non-specific low back pain, postural neck pain, post-surgical reconditioning (orthopaedic, abdominal, breast surgery), pelvic floor dysfunction, scoliosis, osteopenia and selected sports injuries.

Evidence shows moderate benefit for chronic low back pain when delivered by qualified instructors and combined with education and progressive loading; benefits over other forms of structured exercise are similar.

Safety considerations include screening for red flags, individualisation in osteoporosis (avoiding flexion and rotation), pregnancy modifications, gradual progression, attention to breath holding and avoiding pain provocation.

When to See a Doctor?

  • New severe back, neck or pelvic pain with red flags

  • Known osteoporosis before starting flexion-based exercise
  • Pregnancy with obstetric complications considering Pilates
  • Recent fracture, surgery or unstable cardiac disease
