What is Physical Therapy in Rheumatic Diseases?

Rheumatic diseases include rheumatoid arthritis, ankylosing spondylitis, psoriatic arthritis, systemic lupus erythematosus, and osteoarthritis and share inflammation, pain, and progressive functional limitation. Physical therapy addresses joint stiffness, muscle weakness, and deconditioning that arise from inflammation and inactivity.

Therapeutic exercise is graded to disease activity. Land-based aerobic exercise such as cycling and aquatic therapy improve cardiovascular fitness without joint loading. Range-of-motion and stretching programs prevent contractures and resistance training preserves muscle strength around inflamed joints.

Adjunctive modalities include superficial heat for chronic stiffness, cryotherapy for acute flare, transcutaneous electrical nerve stimulation, and ultrasound. Patient education on joint protection, pacing, energy conservation, and orthotic use sustains long-term self-management. Multidisciplinary collaboration with rheumatology and occupational therapy optimizes outcomes.
