Lymphedema

What is Lymphedema?

Lymphedema is a chronic edema caused by accumulation of protein-rich lymph fluid in the tissues due to structural or functional insufficiency of the lymphatic system. It has primary (congenital) and secondary (acquired) types.

Secondary lymphedema is most often seen in Türkiye and developed countries after breast cancer surgery and radiotherapy. Lymph node dissection and radiotherapy damage the lymphatic drainage pathways.

Complete decongestive therapy (CDT) is the gold standard for treatment of lymphedema and consists of manual lymphatic drainage, compression bandaging, exercise and skin care.

Symptoms

Progressive heaviness and swelling in the arm or leg
Tightness of rings or watches
Skin hardening and fibrosis (in the chronic phase)
Skin redness and induration
Recurrent skin infections (cellulitis)
Fatigue and movement restriction

Risk Factors

Surgery for breast, prostate, uterine or melanoma cancer
Lymph node dissection
Radiotherapy
Obesity
Tropical filariasis (the most common primary cause worldwide)
Trauma and recurrent infections

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • New swelling in an arm or leg after cancer treatment
  • If swelling is accompanied by fever and redness (cellulitis)
  • If the swelling is increasing rapidly
  • When there is no response to conservative treatment
  • When daily activities are restricted
