What is Peroneal Neuropathy (Foot Drop)?

Peroneal neuropathy most commonly arises from compression of the nerve at the level of the fibular head, where it lies near the surface. Conditions such as crossing the legs, prolonged squatting, tight casts, and surgical positioning on the operating table can compress the nerve.

The classic presentation is 'foot drop'; the patient cannot lift the ankle upward and cannot walk on the heels. While walking, the toes catch on the ground and the patient must lift each step high.

Diagnosis uses physical examination, electromyography, and imaging when needed. Conservative or surgical treatment is planned based on the underlying cause; early diagnosis improves the chance of functional recovery.

Symptoms

Inability to lift the ankle upward
Inability to lift toes upward
Toe scuffing while walking
Numbness on the outer foot and top of toes
Inability to walk on heels
High-stepping 'rooster gait'
Burning and tingling on top of foot

Risk Factors

Prolonged squatting or sitting cross-legged
Habit of crossing legs
Excessive weight loss and muscle wasting
Diabetes and metabolic neuropathies
Trauma and fractures around the knee
Use of casts or tight bandages
Prolonged positioning after surgery

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If you cannot lift your ankle
  • If your foot scuffs when walking
  • If you have spreading numbness on top of the foot
  • If falls and stumbling have increased
  • If weakness developed after trauma or surgery
