What is Peri-Implantitis?

Peri-implantitis is an inflammatory disease of soft and hard peri-implant tissues characterized by bleeding on probing, suppuration, increased probing depth and progressive marginal bone loss beyond expected remodeling.

Diagnosis requires bleeding/suppuration on probing, increased probing depth compared with previous examination and radiographic evidence of bone loss. Risk factors include history of periodontitis, smoking, poor plaque control, diabetes and prosthetic factors.

Differentiate from peri-implant mucositis, which lacks bone loss. Untreated peri-implantitis often progresses non-linearly and can result in implant loss.

Symptoms

Bleeding on probing around the implant
Suppuration or pus discharge
Increasing probing depth
Mobility (late stage)
Gingival redness and swelling
Halitosis or bad taste

Risk Factors

History of periodontitis
Smoking
Poor oral hygiene and plaque control
Uncontrolled diabetes
Cement remnants under the prosthesis
Inadequate keratinized mucosa around implants
Implant overload from occlusal forces

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Bleeding or pus around an implant
  • Increasing pocket depth on examination
  • Implant mobility or pain on chewing
  • Radiographic bone loss between recalls
  • Recurrent halitosis localized to the implant
