What is Pediatric MRI Without Anesthesia — Feed and Wrap Technique?

Feed and wrap is a non-pharmacologic technique that uses post-prandial natural sleep in young infants to obtain diagnostic-quality MRI without sedation or general anesthesia. It is most effective up to about 4-6 months of age, when the infant has predictable feed-sleep cycles and limited spontaneous movement.

Preparation includes a controlled fasting period followed by a generous feed, swaddling with appropriate hearing protection, careful temperature control and immobilization devices such as MedVac bags. Optimized fast MRI sequences (single-shot, motion-resistant techniques) and a quiet, dim environment increase success rates above 80-90%.

Advantages include avoidance of anesthetic-related neurodevelopmental concerns, shorter recovery, reduced cost, and earlier discharge. Limitations include the need for highly cooperative infants, longer set-up, and a fail-back plan with rescue sedation for incomplete studies.
